Why every incident needs a written report
A guest slips on a wet step, a phone goes missing from a room, a child feels unwell after dinner, a short circuit trips the power on a floor. Whatever the incident, the first few hours decide how well the hotel can help the guest and protect itself. A written report made the same day captures the facts while memories are fresh, shows that staff acted responsibly, and gives management what they need to prevent it happening again.
What a good incident report contains
- Type and severity of the incident, so serious cases are escalated quickly.
- Exact date, time and location, for example "main staircase, last step" rather than "staircase".
- People involved and witnesses with room numbers and contact details.
- A factual description of what happened, in the order it happened.
- Action taken: first aid, doctor, police, repairs, and what was offered to the guest.
- Signatures of the person reporting, the duty manager and the general manager.
How to use this form
- Choose Filled report to type the details now, or Blank form to print copies for the front desk and each department.
- Pick the incident type. The follow-up checklist changes to match it, for example preserving CCTV for theft or keeping food samples for a food complaint.
- Write only facts. Avoid opinions, blame or statements that admit liability.
- Print, sign and file the report, and attach photos, CCTV notes, doctor's notes or a police reference where relevant.
Learning from incidents
Review all reports every month. Repeated falls on the same staircase point to lighting or flooring; repeated thefts point to key control. Turn the lessons into written procedures with the hotel SOP generator, and make sure safety and fire licences are current using the hotel license checklist.
In an emergency, call for medical help, the fire brigade or police first. The report comes after the guest and staff are safe.

Frequently asked questions
When should a hotel incident report be written?
As soon as the situation is under control, ideally within the same shift. Facts, times and witness details are most accurate when written immediately.
Should the guest sign the incident report?
The guest may add a statement if they wish, but they are not required to sign. The staff member who reported it and the duty manager should always sign.
How long should incident reports be kept?
Keep them for as long as your management, insurer or legal adviser recommends, and longer for injuries or police matters. Store them securely because they contain personal details.
