Guest Feedback Form Generator
Create a printable A4 guest feedback form with the sections you choose, a rating scale with tick boxes, a recommend (NPS) question and comment lines.
Why collect guest feedback on paper
Online reviews tell you what guests say in public — often after it is too late to fix anything. A short in-house feedback form catches problems while the guest is still with you, shows guests you care, and gives you a steady record of what is working by department. This free generator creates a clean, printable A4 form with tick boxes, comment lines and guest details. Choose the sections and rating scale that suit your property.
How to design a form guests will fill
- Keep it short. Two minutes is the limit for most guests. Remove sections that do not apply to you, such as food if you do not serve meals.
- Use one scale. Words (Excellent to Poor) are quicker for leisure guests; numbers (1 to 5) are easier to average in a spreadsheet.
- Add one open question. "What can we improve?" often gives the most useful answers.
- Ask the recommend question (NPS). The share of guests scoring 9–10 minus the share scoring 0–6 gives your Net Promoter Score, a simple loyalty measure you can track monthly.
When and how to hand it out
- Place it in the room on the second night for longer stays, or hand it over at check-out.
- Offer a pen and a box at reception so it can be dropped in privately.
- For guests who score you highly, show them your Google review QR card — create one with the Google review QR code generator. Ask every guest equally; do not offer rewards for reviews.
Turn feedback into action
Enter the scores weekly in a simple sheet, look at the lowest-rated questions and fix one thing at a time. Share the good comments with the team — it lifts morale. For negative online reviews, use the review reply generator to respond professionally.

What is NPS in a hotel feedback form?
Net Promoter Score asks guests how likely they are to recommend you from 0 to 10. NPS equals the percentage of 9 and 10 scores minus the percentage of 0 to 6 scores.
Can I edit the questions?
